Overview
Add Form Template Element
Form Template Elements are added to a Form Template using the drag and drop method.
Edit Form Template Element
Properties for a Form Template Element on the Form Template can be reviewed and modified using the Properties Icon.
The properties are specific for each Form Template Element type.
Delete Form Template Element
A Form Template Element on the Form Template can be deleted using the Delete Icon.
A confirmation message will be shown that requires action.
Save
When changes are made to the Form Template Elements the Form Template should be saved using the Save button.
Changes that can be made are:
- Add Form Template Element
- Delete Form Template Element
- Modify Form Template Element Properties
Back
The Back button will navigate back to the Form Template List.
A confirmation message will be shown that requires action.
Form Template Element Types
There are a number of different Form Template Elements that can be used when designing or defining a Form Template.
A Form Template can have multiples of the same Form Element type.
Each of the Form Elements are described below.
Text
The Text field is a text-box that accepts a single line of text.
The Text field is useful for collecting one-word answers and short phrases.
FIELD DESCRIPTIONS
Field | Description |

Title | Label Text for the Form Template Element. |
| Width | Set the width of the Form Template Element as a percentage of the Form Template width. Options are:
The Custom option displays a field for entry of the custom value.

| Mandatory | Set the Form Template Element as mandatory at the time of field creation by selecting the Yes option. Mandatory Form Template Elements will require a value before the Form Template can be submitted. |
Multiple Column | Set the column positioning of the Form Template Element on the Form Template. Options are:

Paragraph Text
The Paragraph Text field is a large text-box that accepts multiple lines of text.
The Paragraph Text field is useful for collecting lengthy answers and long phrases.

Number of Rows | Set the number of multiple lines of text for the Form Template Element. This field can be left blank. |

Multiple Choice
Multiple Choice is a closed-ended field type where Users can select only one answer from a list of choices.

Checkboxes is a closed-ended field type where Users can select any number of answers from a list of choices.

Choose from a list
Choose from list (Drop-down) is a closed-ended field type where Users can select only one answer from a menu of choices.
Choose from list (Drop-down) can be used for presenting a long list of options without cluttering the Form.
