Form List
The Forms are listed as rows in a grid.
The grid displays the following data columns:
- Form Title
- Description
- Request Template
The right most column contains the Action Icons that are used to perform various functions for each Form.
Search
A keyword search of the Form Name which allows the User to refine the Form displayed in the grid.
A keyword search looks for the word(s) anywhere in the Form Names. Keyword searches are useful when the User has part of the information.
Tabs
Tabs help organize the views within the Form context.
The following Tabs are available:
- Approved
- Unapproved
- Disabled
Selection of a Tab will result in the Form information displaying accordingly.

FIELD DESCRIPTIONS
Field | Description |
---|---|
Title | The name of the Form. Form names must be unique. This is a mandatory field. |
Description | Description of the Form. |
A4 Size | Sizes Forms that can be printed. |
Preview
Opens a preview of the Form in another Browser Tab to enable the User to visualise how the form will appear.
Edit
Opens the Form in the Edit mode.
All fields can be edited.
View Answer List
Displays a list of all the Requests that have been created for the Form. For each Request the list shows:
- User
- Request Id
- IP Address
- Filled Date
View
View and download the completed form in a PDF format.
Map
This Action Icon is only enabled only after the Form is Approved using the Approve Action Icon.
A Form can be mapped with a Request Template.
One Form can be mapped with one Request Template.
If a Form has been Mapped the Icon will be colored GREEN.
FIELD DESCRIPTIONS
Field | Description |
---|---|
Request Name | The name of the Request. This is a mandatory field. |
Process | This information is associated with Processes managed in the Process menu. This is a mandatory field. |
Request Template | This information is associated with Process Templates managed in the Process menu. This is a mandatory field. |
Client | This information is associated with Clients managed in the Client menu. This is a mandatory field. |
Form Email Template (Send Email) | The Email Template used for manual Form notifications. This information is associated with Form Email Templates managed in the Form Email Template menu. |
Authenticate Form | The Form will require authentication:
Only one option can be selected. |
Allow to Submit by Staff User | The Form can be submitted by Staff roles on behalf of Students:
Only one option can be selected. Staff are managed in the Configuration > Users menu. Students are managed in the Client > Client Users menu. |
Allow Applicant To Edit Submitted Form | The Form can be edited after submission:
Only one option can be selected. |
Restrict Submission On Existing Request | The same User cannot submit the Form again until any previous submissions of this Form (Requests) are completed, rejected, or made inactive:
Only one option can be selected. |
Form Email Template (Notify for Acknowledgement) | This information is associated with Form Email Templates managed in the Form Email Template menu. |
Map Roles | Sets the User Roles that are able to submit the Form. This information is associated with Roles managed in the Configuration > Roles menu. The Roles will apply to Users managed in the Configuration > Users menu and the Client > Client Users menu. |
Form Group | Reserved for future use. |
Form Submission Options
Mapping configuration for the following Form Submission options:
- Student only can submit Form. Form is visible to Student
Allow to Submit by Staff User = No
Map Roles = Student
- Student can submit Form or Staff can submit Form on behalf of the Student. Form is visible to Student.
Allow to Submit by Staff User = Yes
Map Roles = Student
- Staff can submit Form. Form is not visible to Student
Allow to Submit by Staff User = No
Map Roles = Staff (e.g. Teacher)
Approve
Once a Form has been Approved it can be Mapped with a Process Request Template.
A confirmation message will be shown that requires action.
If a Form has been Approved the Icon will be colored GREEN.
Unapprove
Once a Form has been Unapproved a Mapped any Process Request Template will be Unpublished.
A confirmation message will be shown that requires action.
If a Form is not approved the Icon will not be colored.
Disable
Once a Form has been Disabled any Mapped Process Request Template will be Unpublished.
A confirmation message will be shown that requires action.
If a Form has been disabled the Icon will be colored RED.
Enable
Once a Form has been Enabled it can be Approved and Mapped with a Process Request Template.
A confirmation message will be shown that requires action.
If a Form is enabled the Icon will not be colored.
Delete
A confirmation message will be shown that requires action.
The Form must have been Mapped to be able to send the email
Click Compose Email if the mapped Form Email Template is not to be used.
Clone
A confirmation message will be shown that requires action.
