USI Request

When student login to the system, He/she can see the USI Request icon on the Homepage of the student Portal.
Upon clicking on this icon it will take to the USI consent form as below, which needs to be filled up by student and submit it. This allows staff to generate the USI on behalf of student.

Steps to follow to submit USI consent form:

1. Enter the birth Place
2. Select the DVS Document type
3. Upload the ID document and click on the submit button.
